SOLUTION

Paragraph one: In my experience, I’ve worked in both a blame culture and a culture of safety within different departments of the same organization. For instance, in the surgical department, there was a prevalent blame culture where mistakes were often met with punishment or reprimand. An example of this was when a nurse accidentally administered the wrong medication due to a confusing label, instead of addressing the root cause of the labeling issue, the focus was on reprimanding the nurse. Conversely, in the quality improvement department, there was a strong culture of safety where mistakes were viewed as opportunities for learning and system improvement.
