INTRODUCTION

As changes are occurring in the fields of healthcare financing, policy, and focus, more emphasis is being placed on healthier communities. One of the primary roles of a community health nurse is to assess the community or population in order to determine its health status in relation to its assets and needs.

In this task, you will engage in a Sentinel City Community Health Simulation, a virtual city representing what you may find in any actual United States city. You will be able to take a virtual bus trip and a walk around the city while observing the environment and people of Sentinel City. Through your observations, you will gather information to assess potential health issues and risks within this online virtual learning environment.

You will assess different factors that may contribute to or detract from the health and wellness of the community and its citizens. During your trips through the city, you may be asked to describe the people you see and any expected or unexpected findings related to the citizens of Sentinel City. Your keen observations in this simulation will be the foundation to successfully completing assignments within your course. Record your observations of Sentinel City in preparation for this task.

The link to the Sentinel City Community Health Simulation is located in the course materials, Unit 4. Please review the “Overview Video of Sentinel City Simulation” and “Quick Start Guide” in Unit 4 before beginning the simulation.

You will need to use the “Demographics Assessment,” “Neighborhood Safety Inventory,” “Windshield Survey,” and “Population Health Scavenger Hunt” exercises, attached below, to complete part B of this task.

REQUIREMENTS

Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The similarity report that is provided when you submit your task can be used as a guide.

You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.

B.   Complete a needs-assessment summary to determine the health needs and risks of the virtual community in Sentinel City by using each of the following attached tools and include a brief summary of how each tool was used in your needs assessment:

•   “Demographics, Neighborhood/Safety, and Scavenger Hunt Assessment Tools ”

•   “Windshield Survey”

1.   Analyze the collected data from part B, using concepts of epidemiology and health determinants.

Note: Refer to these three sections of your course for additional information: Epidemiology in the Community, Public Health Data, and Community Health Assessment.

C.   Formulate a community diagnosis for Sentinel City by doing the following:

1.   Discuss three problems you identified in your assessment of Sentinel City in part B, and provide a corresponding Healthy People 2020 goal (see the “Healthy People 2020“ web link) to support each problem.

2.   Identify a primary prevention topic based on one of the problems you discussed in part C1.

Note: See the attached “Primary Prevention Topic List” for guidance.

3.   Discuss two community resource sites (e.g., Sentinel City Affordable Housing Project, Better Health Clinic) that are currently available to address the primary prevention topic identified in part C2.

D.   Discuss how you will apply the assessment strategies learned from the simulation environment to an assessment of your own community.

Demographics Assessment

The demographic assessment of Sentinel City covers various aspects of the city including its population, household income, age breakdown, race, poverty level, and the percentage of the insured and non-insured. With regard to the population, Sentinel City has a total population of 663,862. The city’s median household income is $49,091 and as of 2021, 18.9% of the city’s resident lives below the poverty line. With regard to the age breakdown, 7.4% and 21.7% of the population are 5 years and 18 years and below, respectively. Individuals above 65 years’ accounts for 10.5% of the Sentinel City population. Lastly, with respect to race distribution, Caucasians (whites) are the majority, accounting for 80.6%.
